Description or abstract of dataset: While summary of the existence of US state policies regarding physical education and physical activity are available, measures of the policy implementation are rare therefore, the US-PEPAP implementation survey was designed to capture accurate and up to date information on physical education and physical activity policy implementation in schools as reported directly by physical educators. A secondary purpose of this work was to engage physical educators, university researchers, and other proponents of lifelong physical activity in becoming advocates for more favorable policy in each of the 50 US states. 3. Description of methods used for collection/generation of data: The data were collected by first constructing a stratified sample of potential recipients in each US state for distribution of an electronic survey. These recipients were confirmed as current physical education teachers, and the sample was stratified by school level data such as percentage of students receiving free or reduced lunch, locale (rural, urban, etc.), and geographic location (in-state). An electronic questionnaire was then distributed to participants with questions requiring participants to indicate factual data about topics such as frequency of physical education / adapted physical education class and recess offerings, length of those offerings, class sizes, and policies regarding withholding physical education or recess. In addition, questions about school implementation of waivers, substitutions, exemptions, and online offering of physical education were included, as were estimations of physical activity opportunities offered before and after school. The US-PEPAP questionnaire included 18 policy items adapted from the Physical Education and Activity District Questionnaire – Public Use Version (Roberts, 2016), and the Foods and Physical Activity in Public Elementary Schools: 2005 Questionnaire (Parsad, 2006) and 12 demographic questions. A similar version of the survey was used successfully for a statewide survey in Wyoming in 2021 (see Kern et al., 2021). Kern, B.
